Witten likely was the one who missed the block on Vincent's blocked field goal. Witten blocked the outside guy, and Colombo blocked the inside guy, leaving Vincent to come free right between them. Normally, you block down and make the outside guy take the longest route to the ball.

Colombo didn't block the inside guy, he didn't block anybody, he just ran into the back of the offensive lineman who was next to him. He was like a dog trying to hump the nearest leg he could find. He was a complete waste on that play.
