Great list!
I would add a few guys who may or may not have met your criteria but certainly deserve consideration:
Cowboys:
Don Perkins - Perkins retired as the #6 all-time rusher and was an all-pro/pro-bowl selection several times while playing on some lousy teams.
Falcons:
Jeff van Note - van Note was one of the best Centers in the league for many years but again played for some lousy teams so he was overlooked.
I've got to run but I thought I would throw those two names out now and add some more later when I have time to do a little more research.
