Hey guys, myself and another member on this forum have a friend who graduated from high school with us, who needs your thoughts and prayers. Thought I'd share it here, since I know the Zone will support him.

This is from another one of our friends who knows about the situation. Thanks in advance.


I went to see him this morning. For some of you, the info below will be a shock because you know nothing and I'm sure for some of you, the info below will be old news. But, here's the update as of noon:

His mom said on Monday morning, he got up, loved on the dog and went to the bathroom. It wasn't five minutes later he came out complaining of his right side hurting and said he was going to the hospital. Initially, they told him they thought it could be kidney stones. Through Monday, it got worse and by Tuesday morning, they put him on a ventilator. The final diagnosis, unfortunately, is that he had an aortic aneurysm.

Yesterday, they tried to wean him off of the ventilator by moving it down from 100% to 70%. It didn't go well, so he's back at 80% today. They have him purposefully paralyzed in order to avoid movement with the ventilator. The nurses believe he can hear when you speak to him but he's in a really groggy state and ideally, they don't want him awake for obvious reasons. His mom reported some brain swelling, but the nurses indicated he has no neuro consult, which to the nurses here at my office, means they aren't as concerned about his brain. His mom also reported he had a collapsed lung. I saw no chest tube so I'm not sure how they are treating that if that’s the case, but he does have a pulmonologist, and the nurses did indicate they are concerned with lung function.

They are also extremely concerned about his kidneys. He is having some urine output, but not enough. So, he's got a nephrologist on his case who had not been in yet this morning. Additionally, one of the bigger issues on top of all of this is that his blood pressure is all jacked up. They are trying to keep it as close to 120 without going over as possible. It's been around 111 to 112.

They've also had a problem getting his feeding tube in. Because of the particular bed that he is on, it would require a great deal of agitating movement to get the imaging necessary to use the video device to correct it, but it looks like they may have to do that to start his nutrition. I'll know more about that this afternoon when I get up there because when I left, they were doing the imaging they could do to determine if they'd been able to place it manually.

Back to the aneurysm…they are trying to stabilize him and treat it medically at the moment but he does have a cardio thoracic vascular surgeon checking on him to determine when and if surgery is necessary. Main concern there is again his blood pressure and of course, bleeding so they are trying desperately to avoid surgical interventions.

He needs all the help he can get so please keep praying to whichever higher spirit you believe in, thinking happy thoughts, etc.

^^^ Co-sign. That's terrible. I couldn't imagine going through something like that. I hope his condition improves quickly...and fully.

aortic anuerysm- most people never even make it to the hospital.
He has already beaten the odds big time. If he can hang in there another few days he has a real shot.

I don't know whole lot about aortic anuerysm, but it sounds very serious. My prayers with him and his family and hope he gets well soon.

Thanks for the thoughts everyone. I have two updates to post.

This was early Friday...
...went to see him again after work yesterday and a couple of things had changed, including my perception of the diagnosis. He does have an aortic aneurysm but he also has an aortic tear. One is an issue, a combination of the two is obviously a bigger one. But, alas, there is some good news too...his nephrologist (kidney doctor) had been in yesterday and the nurse said he hadn't ordered dialysis which is good so they're hoping his kidney function has improved on its own. His blood pressure was also better and it was holding without the use of one of the medicines they weaned him off of successfully yesterday. His sister and the nurse said they would basically try to stabilize him and watch to see what they needed to do with regards to the tear and the aneurysm without surgery…and the nurse seemed hopeful there was a good chance of not needing the surgery but also said that changes day to day depending on his condition.

So, overall it seemed he had a better afternoon yesterday. His sister also mentioned that initially the doctors told them he would probably be on the ventilator for a few days or so to let his lungs "rest" but that they may try to wean him again so he's off in the next couple of days if he's doing well.
And the second update, from Friday afternoon.

Overall, some improvement since yesterday…

First, the bad news….he has pneumonia. Good news is that the doctors are confident they caught it early and that it will be easily treated with antibiotics.

He has fluid building up around his lungs that they are trying desperately to get rid of…most of it stems from the fact that his kidneys aren't properly functioning so they have him on strong medicines to help with his urine output. Good news is they seem to be working. If it doesn't work well enough today, they will put him on dialysis tomorrow but they're trying to hold off on that. So, really pray his kidney function improves a lot over the course of the next few hours!

They had him down to 65% on the vent and he was doing really well on his own but because he's coughing up so much fluid because of the kidney medicines it was making it harder to breathe on his own. But, they are going to slowly take him off of the paralytic medicines this weekend and if it goes well, the doc hopes he'll be communicating a little by Sunday.

His mom and sis said he'd been more reactive this morning. Said he was squeezing his hands when they held them and would react with his eyes when the called his name. He wasn't doing that when I was in there but the docs were messing with him and taking his arterial blood gases so I was actually glad he wasn't reacting when they were sticking him because his best friend Ashleigh, said taking the arterial blood gases was terribly painful since they go past the veins to the arteries. The doc told his mom and sis that people should start stroking his hands lightly and talking really calmly to him today and tomorrow to slowly get him integrated with the waking world again but to not get him excited or talk about how ill he is. There is some concern when they take him off the medicines he'll be out of it and go a little bit "cwazy" like he did before he went on them. His mom and sis said the nurses were pissing him off because he wanted to go home so he ripped all of his tubes up and pushed a nurse out of his way…not quite the cedrick we know and I know its not funny because of the reasons but the image of it is a little entertaining. On another lighter note, he was trying to show [us] his stuff last night …his gown kept riding up and riding up until I told him it wasn't going to work and we pulled his gown back down.

That's terrible. So tragic how you an be fine one minute and clinging to life the next. My prayers go out.

If it helps any, I knew someone last year who had the exact same thing happen. He was only 35, and had no health problems at all. The situation sounds identical -- thought it was a heart attack at first, etc, but he had the a.a. along with bad tearing. Fought infection the same way. It was touch and go for a few days, but he recovered and is now fine.

Hope this situation turns out the same.

sorry to hear about your friend, a lot of times with an aortic aneurysm the kidneys suffer. sometimes its just acute renal failure and dialysis is needed for a short time.

if he continues on the vent and they cant do the feeding tube someone should mention iv nutrition, TPN (total parenteral nutrition) though its only supposed to be used when the gi tract is not functioning.... due to the expense of the formula you would rather use an enteral formula through a tube feed than a iv feeding, but i used to recommend it when i was the dietitian at a surgical unit if tube feeds were not feasable or if family members refused to consent for a feeding tube placement even if the gi tract was functioning. something to consider.

Sorry to hear it Peplaw. Is this the same thing that got John Ritter/Jonathan Larson? Have they diagnosed him with Marfan's syndrome (ie - is your friend tall/lanky, stuff like that?)

Wiki says Ritter and Larson died from an aortic dissection... I don't know if that's the same thing or not...??

He hasn't been diagnosed with Marfan Syndrome, at least not that I know of. He's about average height and overweight.... not tall and lanky.
