Mine's the simple but effective Cry Baby Wah by Dunlop.

A shining example of what the Wah pedal sounds like is the intro to Voodoo Child, by Jimi Hendrix. Another great example is Clapton's solo in White Room by Cream. And a third is the epic solo to Sweet Child O' Mine by Guns n Roses.

What's your favorite and why?
 
I'd have to say Wah, as well. I have some kind of processor (DigiTech?) thing that makes cool noises and all that, but in the grand scheme, I don't care for effects and UFO sounds. When I played in a band I just went through the wah into the amp, nothing else seemed necessary. Harmonies were just handled with 2 guitarists.

I loved the wah, but somewhere along the way I lost it and I have no idea when or where...
 
I had a Hendrix Cry-Baby back in the late 80s. Right now I have a DigiTech RP350. I basically play most of the time without it and straight distortion on my Line6 amp.
 
I bought a modeling amp recently (Fender Mustang II) and since then all of my pedals except for my Dimebag Wah are gathering dust. That pedal is just killer. Everything else I could ever imagine is handled by the Mustang...what a fantastic amp.
