Is the penalty intentional grounding or just grounding? Because in the past I've seen QBs throw a ball deep when a WR cut off the pattern and a flag not thrown. Does "intent" have anything to do with that penalty?

It was a bad call by the refs.
There have been many passes thrown through the end zone as throw away passes, as well as miscommunications.
I do not recall any of them being flagged.

It is supposed to. It is a penalty if the throw is made to avoid pressure.
