4 TEs Formation (What game was it?)

Quick Question guys. I am doing a breakdown of Kellen Moore's offense (it's been a lot of fun). I am in the middle of going through film and I just remembered a game when we took a snap or 2 with 4 TEs in the game.
I'll obviously bump into it eventually, but I wanted to see if anyone remembered which game / scenario?

I didn't know we had 4 TE on the roster.
 
In the Carolina game, the goal-line play at 7:35 in the 1st quarter, which resulted in an incomplete pass to Schultz, featured 4 TEs. They talked about it in the booth; Greg Olsen got very excited. Schultz and Jarwin split out left, Sprinkle split out right and someone, I think must have been McGovern but I can't quite tell, played in the normal TE spot on the right side of the line as an eligible receiver.

It was Schultz, Jarwin, Sprinkle, and McGovern reporting as eligible.

I hope we see McGovern get a TD this year. Fat guys getting TD's means we get to see a fat guy spike, a fat guy celebration
Nothing better! I'm waiting on the wrinkle that gets him a TD. I'm also waiting on the "CeeDee lined up at HB" wrinkle. Think they settin something up
