How much is due to bad luck vs. conditioning issues, etc. due to limited offseason? Something else?

Yeah a combination of the two. None of these players were truly ready for the season.

All the whiners about the "boring" preseason should know now there is a real purpose for it. If you're bored don't watch.

I don't necessarily buy the preseason thing. Starters rarely play much in those games anyway. Why? To avoid injuries. In my opinion preseason has never been a time where the players get tuned up and ready to go and its been that way as far back as I can remember. Preseason generally has been about getting a look at the rookies and others trying to earn spots on the team. Starters already have their spot solidified, which is another reason you rarely see them play in those games.

In Zeke's 4 plus seasons, how many preseason snaps has he taken? How about Dak?

The gradual progression to playing a real game being important must be considered. IMO.

Starters may not play much in preseason games but they are practicing a lot and they hit in camp.
Playing college football it didn’t matter how good a shape you were in the first couple days of hitting you are sore as hell. Your body needs to get used to it and ramping up helps. We’d work into going live, couple days helmets only, then uppers, then full go. The ramp up allowed you to get into football shape. For lots of guys this year the first life action was week 1, that’s going to lead to soft tissue injuries.

We defintly were not prepared to start the season from a conditioning pov, how much of that lead to the injuries and how many of the injuries were preventable is debatable

Combination of lack of prep, and while I think preseason games are useless these players didn't go through a regular camp and prep, and some conditioning could be the culprit.

Lee, Vander Esch, Big Smitty were not surprise injuries.

The Cowboys are not built for depth, they are a team that will succumb to injuries and it doesn't take many.
