Penalties - Still a problem - 4 years later

Does anyone remember when MM said he was going to fix the penalties in the off-season after the first loss to San Fran in the playoffs?

If I was Jerry. That is grounds for dismissal. Right there. You had all year and you are going to fix it in the off-season?

Well here we are 4 years later, guess what? Penalties are still a MASSIVE issue with this team.
