Let's say a RB is to run a wheel route. As he's running the route, he trips in the flats. The QB, under pressure, throws the ball 10 yards beyond the LOS, where there's no receiver in the vicinity.

And how does the ref know the design of the play?
all the ref can examine is the actual play that unfolds before his eyes. unless you provide the ref with the playbook, all he's got to go on is where the ball lands relative to the eligible receivers on the field. he's got no legit ability to say "well the guy who tripped was going to spot X"
