Lewis appeared to screw up and as you said, Heath was not watching for the run, he was watching the slot WR that nobody else was covering.
As you said, Lewis following the slot WR from one side of the formation to the other pre-snap indicates man coverage, but Lewis played zone.
Both outside CBs played man coverage, which normally means the Nickel CB would play man.
People love Lewis, but there are reasons that he is the 4th CB.
Notice how Byron Jones stays much deeper than the WR as compared to Awuzie trying to play tighter against Anderson. Regardless of speed, Awuzie needed to play further off if that's what he needs to stay over the top of the WR when he is isolated in man coverage with no help.

You notice the LBs and Jourdan lewis drop into a zone look. So what does it matter if it was a bad angle or if was his slow reaction and slow feet? The end result is still a 92 yard TD for the other team.

Looks pretty clear cut. Awuzie screwed up and then Heath screwed up too.
They both suck. You don't have to analyze screenshots frame-by-frame to see they both messed up and caused a 90+ yd TD.

He initially starts to transition at the 25 yard line then he sees that the slot WR is clearly not being covered by anybody and Heath then back-peddles to about the 30.
Maybe Heath could have transitioned at about the 28 instead of the about the 29.5.
The QBs arm is going back and the QB is looking at Anderson when Heath is at about the 28.
The QB releases the ball when Heath is at about the 29.5 yard line which is the point the Heath had transitioned.
Lewis was not scorched he was in a zone not playing man to man.He's the single high S, x. The guy is running a shallow crossing route and there is a deeper route on the other side of the field.
Zone defenders are taught to get into their drop and drive on the football. That crossing route is designed to fool him and he was. It's an old old route combo.
