They basically improvise. There have been wrestlers who have been seriously injured and cant continue. The referee and the opponent find some kind of way to finish. Look on youtube at Sid Vicious leg injury, his opponent had to improvise. HHH had a wuad injury with DX and Shawn Michaels had to be DQ for no reason.
